Your average cost is everything you paid, fees included, divided by the coins you actually hold:
average cost = total USDT spent / coins received
On Binance the buy fee is normally taken from the coins you receive, so 0.1% of each buy never reaches your wallet. If you pay fees with BNB instead, you receive the full amount but the fee is an extra cost. Either way the calculator counts it.
Example: you spend 500 USDT on BTC at 60,000 and another 500 USDT at 50,000, with a 0.1% fee taken from the coins. You receive 0.0083250 + 0.0099900 = 0.0183150 BTC for 1,000 USDT, so your average cost is about 54,600 USDT per BTC, not the 55,000 midpoint: buying more at the lower price pulls the average down.
Break-even: selling also costs a fee, so to get your 1,000 USDT back you need a price slightly above your average cost: average cost / (1 - fee).
